What does this test measure?
It measures the cardiovascular response to progressive exertion (heart rate, blood pressure, symptoms) and, when combined with gas analysis, direct VO2max. In training contexts without gas analysis, it's mainly used to estimate VO2max from the total time the subject lasts on the treadmill.

Step-by-step protocol
The Bruce protocol consists of 3-minute stages, increasing speed and incline at each one:

- Start the protocol at stage 1 and advance every 3 minutes according to the table
- Record heart rate, perceived effort (Borg scale), and any symptoms at each stage
- The test ends due to voluntary exhaustion or stopping criteria (in clinical contexts: ECG changes, angina, blood pressure drop, etc.)
- Record the total time reached to estimate VO2max using the corresponding formula (specific equations exist for the standard or modified Bruce protocol)
Common mistakes
- Using the standard protocol (stage 1 is already demanding) for very untrained people → better to use the modified Bruce protocol, which adds two gentler initial stages
- Not adequately monitoring symptoms in populations with cardiovascular risk factors
- Not calibrating the treadmill incline correctly
