Body Composition and Health: The Complete Guide to Tests for Trainers and Physios

A complete guide to body composition and health marker tests: skinfolds, bioimpedance, BMI, circumferences, metabolic age and more. How to choose the right one and what each result means.

If posture tells you "how someone moves and holds themselves," body composition tells you what they're made of and how their underlying metabolic health looks. It's probably the most requested category of tests among training and nutrition clients, and also the one that's most often done badly: scales used without controlling conditions, BMI interpreted out of context, circumferences measured at different points each time...

In this guide we've put together the most relevant body composition and health tests, explained in a practical way, so you know which one to use in each case and how to interpret the result without falling into the most common mistakes.

Why measure body composition and health?

Beyond the "I want to lose fat" or "I want to gain muscle" that almost everyone brings to the table, these tests give you:

  1. An objective baseline for designing the training and nutrition plan
  2. Relevant health data (cardiovascular risk, fat distribution) that goes beyond aesthetics
  3. Evidence of progress that doesn't depend on "how I look in the mirror today," which varies a lot day to day

The tests in this category

TestWhen to use it
Body composition test (skinfolds)You want an accurate, affordable estimate of body fat %, with tracking by the same evaluator
Bioimpedance testYou have a BIA scale/device and want quick, repeatable tracking at home or in the gym
BMI testYou need a quick population-screening or general-context figure (not for muscular athletes)
Metabolic age testYou're looking for an easy-to-communicate motivational figure, derived from bioimpedance
Basal metabolic rate testYou're designing or adjusting a nutrition plan and need to estimate caloric expenditure
Bone density test (DEXA)Your client has access to a DEXA scan (high-level athlete, osteoporosis risk) and you want a reference figure
Waist-to-hip ratio testYou want to assess cardiovascular risk associated with fat distribution
Body circumference testYou want the most practical and motivating way to track physical change, month by month

How to choose the right test for your context

You don't need to run all 8 tests on everyone. Some practical criteria:

  • First assessment with a new client? Start with body circumferences + BMI (quick, no special equipment needed) and, if you have skinfold calipers or a BIA scale, add the corresponding body composition test.
  • Is the main goal aesthetic/body composition? Skinfolds or bioimpedance + circumferences are your main combo. Repeat every 4-6 weeks.
  • Does the client have cardiovascular risk factors, or is the goal "health" rather than aesthetics? Prioritize waist-to-hip ratio + BMI as a screening tool, and refer for medical assessment if needed.
  • Are you putting together a serious nutrition plan? Calculate basal metabolic rate as a starting point, and adjust it with each new body composition assessment.
  • Is the client a high-level athlete or at risk of osteoporosis? Recommend an annual DEXA scan as a reference, and use the other tests for tracking in between.
  • Want a figure that's motivating and easy to explain? Metabolic age (if you have a BIA device) tends to drive a lot of engagement.
In practice, the most common and efficient combination for most clients is: body circumferences + a body fat % method (skinfolds or bioimpedance) + BMI for context, repeated every 4-6 weeks, with waist-to-hip ratio reviewed every couple of months.

From scattered numbers to real tracking

The hard part of this group of tests isn't doing them: it's staying consistent and not losing the data. Skinfolds noted in a notebook, weight in one app, circumferences in another... and in the end nobody compares anything properly.

With Movalytics you can log all of these tests — skinfolds, bioimpedance, BMI, circumferences, BMR, metabolic age — in each athlete's or patient's profile, automatically calculate derived indices (BMI, waist-to-hip ratio, body fat % based on formula), and generate progress charts that help you explain real progress, beyond whatever the scale says on a given day.

Frequently Asked Questions

What's the "best" body composition test? None is the best in absolute terms. DEXA is the most accurate but not very accessible; skinfolds and bioimpedance are practical and good enough for tracking if used consistently. The important thing is to pick one (or two) and stick with the same protocol over time. How often should I repeat these tests? Circumferences and body composition (skinfolds/BIA): every 4-6 weeks. BMI and waist-to-hip ratio: every 1-2 months, since they change more slowly. DEXA: annually or as medically indicated. Why isn't my client losing weight even though their circumferences are improving? This is very common: they could be gaining muscle while losing fat, or retaining more fluid due to training. That's why it's important to look at several markers together (circumferences, body fat %, weight) rather than just the scale. Does it make sense to run all these tests on every client? No. Choose the most relevant ones based on each person's goal and context (see previous section). Running too many tests without a clear purpose just adds noise and assessment time without adding more value. Do these tests replace a medical evaluation? No. They're screening and tracking tools for training and nutrition. Any relevant health finding (high cardiovascular risk, suspected osteoporosis, etc.) should be referred to a medical professional.
